　　“违章、麻痹、不负责任”充分诠释了事故发生的原因。“违章”就是不按规章制度，不按操作规程来做事，凭自己的个人主观意识来做事，久而久之就变成了习惯性违章；“麻痹”则是进一步违章，未认真分析工作过程中的所有环节，不认真履行工作流程和规程，总认为自己这么做没什么大不了，自己是对的，图方便、图省事，从而导致了事故的发生；“不负责任”就表现在多方面，管理上的不负责任，监督上的不负责任，具体工作者的不负责任，即管理不到位、监督不到位、执行不到位。

　　4.25事故发生以后，大家的心情都很沉重，没有人员伤亡乃是侥幸中的万幸，大家同事一场，无论在工作中有什么过节或是在生活中好到什么程度，我想没有人愿意这样的事故发生，更不会有人窃喜和埋怨他们。事故已经发生了，给企业带来上千万的经济损失，也给千家万户的家庭带来不可避免的损失。如果要是我。我会感到深深的愧疚和自责，可是事故发生后我们看到的很让人寒心，当事人的脸上满是开心的张狂，还自称自己是什么主角，接见了什么领导，什么领导对她们怎么关心等等，还把责任推的一干二净。当大家对他们都产生反感时，依仗领导撑腰，厉害得不得了。是啊，班组要整顿，但是包括当事人在内的领导们不该整顿整顿自己的思想吗?

　　事故发生的原因：

　　一：对设备及电力线路不熟悉，不知道35千伏进线柜停电以后，上级110千伏过来的电仍然有电。

　　二：安全意识淡薄，没有搞好确认制。在操作以前没有看到35千伏进线柜的来电显示信号。

　　三：违章作业，违章指挥，没有听到上级指令就自行操作。

　　四：设备本身存在缺陷，没有带电打不开柜门的联锁装置。

　　五：作业人员业务水平有限，危险源辨识能力差，既然验电后证明柜子有电，就不应该进行任何操作。

　　六：与同事关系不和，带情绪作业。

　　20xx年4月22下午三点在调试室三楼会议室矿长亲自主持组织采煤战线副科级以上干部观看了20xx年吉林八宝煤矿“3·29”和“4·1”瓦斯爆炸事故警示教育片。视频开播后，片中的场面使我的思想受到了极大的冲击，事故通过声像播放方式再现事故现场，是我有身临其境之感。通过观看视频后让我感到：在事故面前，生命是如此的脆弱；细小的隐患，在管理者的纵容下，直接成了杀人的工具，而那个杀手，就是我们这些所谓的矿领导。

　　对于煤矿管理者，我们要坚定不移的执行好“双七规定”，保护好矿工的生命安全，在如今严峻的安全形式下，我们是“出不起事故”的，安全事故带来的巨大经济损失，甚至企业倒闭，对煤矿企业来讲：“安全才是最大的效益”。

　　一、事故对于人身、对家庭的伤害。

　　一但出了工伤事故，煤矿企业不但失去了创造产值的劳动力，同时还有承担伤者的医疗费、护理费、误工费甚至养老费等费用；一但出了工亡事故，巨额的赔偿、经济处罚和停产整顿甚至关闭所带来的巨大损失，都是目前煤矿企业无法承受的。所以，我们“出不起事故”。

　　在统计的10起安全事故中，其中违章动火与违章指挥的有4起，我们此刻能够把他都归纳为“违章作业”，由此可见，在我们日常的安全管理中，发生安全事故，“违章作业”占的比例是比较大的，也是我们就应警惕的一个主要防范对象。我们乙炔工段的日常检修比较多，尤其是动火作业更是司空见惯，正因为有这么高频次的检修，也就注定我们在日常的工作中要细致，要精心，要把安全防护措施落实到位，要把本质安全坚持到底，这是我们的原则，也是我们的态度。

　　在10起事故中，其中内蒙古吉兰泰发生安全事故已是第二次，这个问题倒是让我思索良久。为什么同一家企业会在短短的时间内连续发生两起安全事故，我觉的主要原因可能和这个企业上上下下的安全意识有莫大的关联。上梁不正下梁歪，领导不重视，基层肯定也小视，因而酿就一个又一个的惨剧，无不让人叹息!记得我以前讲过，做为一个化工厂的工人，要学会不断地从事故中学习教训，总结教训，充实自己的头脑，让自己变的成熟起来，想的要全面，安排要具体，落实要到位，把关要严密，管理要过硬，检查要细致，整改要彻底，纪律要严明，奖罚要公平，这样才能管理好一个工段，才有可能限度地杜绝事故，远离事故的发生。

　　关于“人”的不安全隐患，我觉的比“物”的不安全隐患更可怕，更难治理，虽然我们口口声声地讲要透过不断地培训与教育提高其操作技能，增强其安全意识，但是对于一些本来自律意识较差，个人素质不高的员工有时也真是“恨铁不成钢”。你对他苦口婆心，他全当耳边风，着实让人无奈的很。因此我觉的，目前公司就应采取必要的末位淘汰制，把一些扰乱民心，缺乏职责心，操作技能差的员工进行必要的淘汰，要把金泰变成一个大熔炉，留下的是金子，淘汰掉的是渣子。只有这样，才会不断地提高员工的个人素质与整体素质的协调发展，更有利于安全工作的全面提升。

　　总而言之，透过对《通知》的学习，深深地给我上了一课，做为乙炔工段的一名副工段长，整日给易燃易爆物质打交道，不仅仅多了一些敬畏，更多了一份职责，眼下只有扎扎实实地开展好各项工作，严格遵守操作规程，做好日常检修的安全防护措施的落实，重大危险源的监控，全面提升个人素质，全面推进工段的各项管理向更高的一个台阶迈进!
